We wish to draw readers' attention to significant overlap between this article and one published in the December issue of the Scandinavian Journal of Rheumatology (2004;33:395-9).

The article published in Scand J Rheumatol is an earlier version which differs in certain details from that in Ann Rheum Dis. The "duplicate" submission arose from a misunderstanding among the authors.
